Hand Surgery in Pakistan: A perspective Hand Surgery in Pakistan has daunting challenges and exciting possibilities. As an organised surgical field, it started with the first meeting of Pakistan Society for Surgery of the Hand (PSSH) in November of 1993 at Bahawalpur. Founding members were: Abdul Hakeem Babar, Muhammad Umar, Muhammad Saleem, Tariq Suhail, and Tahseen Cheema. From a humble start, PSSH has grown into a vibrant professional organisation that is ready to meet its obligations and challenges posed by peculiar circumstances and blend of pathology prevalent in Pakistan. Though there are many fine hand surgeons in the country, dedicated hand centers are almost non-existent, Hand and Upper Limb Surgery (HULS) centre CMH LAHORE being the only such center in the country. The load of hand trauma, both industrial and agricultural, is overwhelming, mostly affecting the poorest and the neediest members of the society, thus adding to the social and economic misery. During last quarter century, pathology has changed, ushered by the demographical and anthropological changes in the society. Polio has been replaced with a more menacing and difficult to treat entity – cerebral palsy. Society in general and medical profession in particular are least prepared to handle this challenge. Explosion in population, chaotic traffic, and curse of motorbikes have created an endemic of brachial plexus injuries, crippling the youth for rest of their lives. Resources to meet this challenge are limited and not well organized. Presently, brachial plexus injuries are being managed by heroic efforts of few individual hand surgeons with great sacrifice of time and available resources, inevitably resulting in rationing of medical care. First replantation of a completely severed hand was done in 1988 at Bahawalpur. Now, there are many surgeons and centers, who have the skills to perform replantations. But such surgical feats remain individual dependent with variable success rate and also at the cost of snatching valuable surgical time and resources from other more impending needs. We need better organization and management of our limited resources to meet these challenges. Unsafe and poorly designed agricultural and industrial machinery, compounded by lack of any safety measures and rules and regulations result in some of the most devastating and heart breaking injuries. We are all familiar with young children with amputated hands from fodder cutting (toka) machines. All these and many other issues while posing great challenges to members of PSSH also offer exciting possibilities of doing some thing really good for our patients and for our society. While keeping ourselves professionally updated and training next generation of hand surgeons, we have to convince the society and its political leaders to help us preserve and protect the hands of working and productive populace. For this purpose, we need dedicated centers of hand surgery in every major center of population, well supported by personnel, working facilities and rehabilitation resources. The country needs three or four specialised center for microsurgery to address the replantations and brachial plexus injuries in an organised fashion, and to avoid spreading this kind of highly specialised work and diluting the experience resulting in less than optimum results. Another major focus for PSSH in future should be to develop auxiliary disciplines like hand therapy and occupational therapy. I have all the confidence in the vision and capability of emerging leadership of hand surgery. Past is only a prologue.

Dear Friends and Colleagues,
It gives me immense pleasure and a proud feeling to write this note as president of Pakistan society for surgery of hand. Pakistan Society for the Surgery of Hand since its inception few decades ago has grown slowly but steadily over the years. In the last few years however its growth has been exponential with hand surgeons joining in from all parts of the country. At the same time due to intensive training programmes and regular workshops, the quality of work has improved tremendously at par with any advance country of the world. Large number of scientific and clinical papers are being presented in our country and abroad by our hand surgeons on a regular basis. I am honoured to be the president of such a prestigious society in my country and pray to Allah that it expands and continues to grow with time. Insha Allah it will.
